Tom Maxwell
Tom Maxwell offers a unique combination of 30 years’ experience, knowledge, compassion, and commitment to the healthcare community, including 18 years focused directly on the homecare and hospice technology industry. He is currently serving as Chairman and Co-Founder of Maxwell Healthcare Associates, an elite business consulting firm that specializes in the home health and hospice industry. Tom also serves as the Non-Executive Chairman of the Board for Medalogix– a Nashville, TN-based healthcare solution dedicated to empowering innovative, data science-driven, visit-based predictive analytics and individualized patient care guidance to home health and hospice patients. Tom has also served in senior leadership roles as COO/CSO with Homecare Homebase (HCHB). This technical dedication combined with years serving the country in the medical field as a Navy Hospital Corpsman (Fleet Marine Force) allows for a detailed understanding of industry, caregivers, veterans, and patients.
Jeremy Powell
Jeremy Powell is a veteran executive with over 20 years in Healthcare Technology. He has delivered solutions to commercial and government providers, patients, and families across North America and Europe. Jeremy saw the need for a radically different approach to delivering care for the patients facing advanced illness. This led to the founding of Acclivity. At Acclivity, Jeremy leads the company and ensures that its mission to better support care delivery for the sickest patients is upheld. He aims to achieve this mission by making Acclivity’s technologies and services available to all stakeholders served by healthcare (patients, families, providers). Jeremy is powered by the loves of his life: his wife Adrienne, his sons Isaac and Seth, and his daughter Hannah.
Wes Little
As Chief Analytics Officer, Wes Little leads WellSky’s unified analytics strategy to deliver data-driven insights and innovative solutions. As General Manager of WellSky Home, Wes oversees WellSky’s home health, hospice, palliative care, and personal care businesses. He previously served as director of product management for Kinnser Software, where he oversaw rapid product innovation for the company’s home health electronic medical record software. Wes holds an MBA from the University of Texas at Austin’s McCombs School of Business and a bachelor’s degree in economics from Washington and Lee University.
Tom Maxwell
Dr. Katy Lanz currently serves PCMA (a growing geriatric and palliative medical practice) as Chief Strategy & Product Officer. She was Chief Clinical Officer for Aspire Health and grew it from a start-up company to a multi-hundred million acquisition as the largest palliative practice in the country. Since then, she founded TopSight Partners, an advisory firm for providers, payers, and investors. Katy applies her two decades of clinical and operational successes (including 10 years spent at the bedside of the seriously ill) to her role as an advisor to growing companies and to her position on the board of the National Hospice & Palliative Care Organization.
